Second Dose COVID-19 Vaccine Clinics for Children Ages 5-11

Iowa City Community School District recently issued the following announcement.

On November 29, 2021, we begin the clinics to administer the second dose of the COVID-19 vaccine to students ages 5-11.  If your child received their first dose of the COVID-19 vaccine at one of the ten clinics held at our schools between November 8-18, they are eligible for the second dose of the vaccine.  Please check the date on the back of your child’s vaccine card, which was provided to you when your child received their first dose. This is the date for your child’s second dose. Please visit the school listed below on the indicated date to receive the second dose of the COVID-19 vaccine.  Your child cannot receive the second dose before the date indicated on the back of the card.  Another vaccine consent form must be completed for each child.  Please complete the form and bring it with you to the clinic if possible.

    December 1, 2021, 3:15-6:00 pm

  • Hoover Elementary (1355 Barrington Rd., Iowa City)

    December 2, 2021, 2:15-6:00 pm

  • Garner Elementary (80 Birch St., North Liberty)

    December 6, 2021, 3:15-6:00 pm

  • Shimek Elementary (1400 Grissel Pl., Iowa City)

    December 6, 2021, 3:15-6:00 pm

  • Lemme Elementary (3100 E. Washington St., Iowa City)

    December 7, 2021, 3:15-6:00 pm

  • Horn Elementary (600 Koser Ave., Iowa City)

    December 8, 2021, 3:15-6:00 pm

  • Twain Elementary (1355 DeForest Ave., Iowa City)

    December 9, 2021, 2:15-6:00 pm

Please note, we will NOT be offering the first dose of the COVID-19 vaccine at these clinics.  These are second dose vaccine clinics only for children that received their first dose through our initial clinics.
